By using this benefits enrollment portal you agree to these terms. If you do not agree, do not use the portal — contact your employer or your agent of record to enroll another way.

The portal is an administrative tool that helps employees of Quad Power EMC review benefit options, record elections and waivers, and reach the carriers' own enrollment systems. It is not an insurance policy, an offer of coverage, or a guarantee of eligibility.

• You must be an eligible employee (or an authorized administrator) to use the portal.
• You agree to provide accurate, complete information for yourself and any dependents you enroll.
• Submitting false or misleading information may result in denial or rescission of coverage by the carrier.

When you type your name and submit your elections, you are providing a legally binding electronic signature and consenting to conduct this transaction electronically. We record your name, the date and time, and the acknowledgements you checked. You may request a paper alternative from your agent.

Plan documents, certificates of coverage, rates, and carrier decisions control in all cases. Any summary, rate estimate, or brochure shown in this portal is for convenience only and may contain errors or be out of date. Coverage begins only when the carrier approves and issues it.

Elections and waivers must be submitted within your employer's open enrollment window or your new-hire eligibility period. Late submissions may be rejected until the next open enrollment or a qualifying life event.

The portal links to carrier enrollment platforms and scheduling tools operated by third parties. We do not control those sites and are not responsible for their content, availability, or handling of the information you submit there.

• Do not access records that are not yours.
• Do not share enrollment invite links, admin credentials, or documents with unauthorized people.
• Do not attempt to probe, scan, disrupt, or reverse engineer the portal.
• Do not upload malicious files or unlawful content.

Employers, agencies, and agents with administrative access are responsible for keeping credentials confidential, for the accuracy of census files they upload, for having authority to share employee data with us, and for the content and licensing of any logo or handbook they upload.

The portal is provided "as is" without warranties of any kind. To the fullest extent permitted by law, Benefits For You LLC is not liable for indirect, incidental, or consequential damages, or for any loss arising from carrier decisions, missed deadlines, or information you submitted incorrectly.

We may update these terms or modify, suspend, or discontinue the portal at any time. Continued use after an update means you accept the revised terms.
