1. Who we are
This benefits enrollment portal is operated by Benefits For You LLC ("we", "us") on behalf of your employer, Quad Power EMC. Your licensed agent of record and can be reached using the contact details in the footer of this site.
2. Information we collect
- Identity and contact data: name, work or personal email address, phone number, mailing address.
- Eligibility data: date of birth, gender, employment status, hire date, and employer or group identifiers provided by your employer's census file.
- Dependent data: the name, date of birth, gender, and relationship of a spouse or dependents you choose to include in coverage.
- Sensitive identifiers: Social Security numbers, where required by a carrier to issue coverage.
- Election data: the plans you elect or waive, waiver reasons, coverage tiers, and the electronic signature, name, and timestamp you submit to confirm your elections.
- Technical data: basic log and device information generated when you use the site.
We do not ask you to submit medical records or health history through this portal.
3. How we use your information
- To confirm your eligibility for your employer's benefits program.
- To submit your elections and enrollment information to insurance carriers.
- To generate your enrollment summary document.
- To contact you about open enrollment deadlines, missing information, appointments, and follow-up questions.
- To keep records required by carriers, regulators, and your employer.
We do not sell your personal information.
4. Who we share it with
- Insurance carriers and their enrollment platforms (for example the medical and dental/vision enrollment sites linked in this portal), so coverage can be issued. Those sites have their own privacy policies that govern the information you enter there.
- Your employer, limited to what is needed for payroll deductions and eligibility administration.
- Service providers that help us operate this portal, including our hosting and database provider and our customer relationship management (CRM) and messaging platform, used for enrollment status and appointment scheduling. The CRM receives employee contact details and status, not Social Security numbers, dates of birth, signatures or household enrollment details.
- Legal and regulatory recipients where required by law.
5. Retention
We keep enrollment records for as long as needed to administer your coverage and to satisfy insurance, tax, and recordkeeping obligations — generally the duration of coverage plus any period required by applicable law. When records are no longer needed, we delete or de-identify them.
6. Your choices and rights
- You may request a copy of the information we hold about you.
- You may ask us to correct inaccurate information.
- You may ask us to delete information we are not required to retain.
- You may opt out of marketing or reminder messages by replying STOP to a text, using the unsubscribe link in an email, or contacting your agent. Transactional messages about your enrollment may continue.
Providing your information is voluntary, but we cannot enroll you in coverage without it.
7. Consent
Before you begin enrollment, the portal asks you to affirmatively agree to this notice. We record the date and time of that consent along with your enrollment record.
8. Cookies and tracking
This site uses only the cookies and local browser storage needed to keep your session and your in-progress enrollment state. We do not use advertising cookies or cross-site tracking. For details, see our Cookie & Tracking Notice.
9. Children
This portal is intended for use by employees. Dependent information, including for minors, is submitted by the enrolling employee or their guardian.
